CTI is a short-term model that provides guidance for patients during their transition from hospital to home. CTI’s concept is to activate patient engagement in their health management. The model is created to function as an information portal where users can access healthcare-related material that fit their needs, apply for training classes or stay notified for CTI’s upcoming events.
